Outcome Measure

Hello,
Can anyone recommend an Outcome Measure to use in acute care for low level geriatric patient?

When you say, “low level geriatric” pt, what does that mean to you? Gait speed is what I perform with all ambulatory geriatric patients in acute care. Non ambulatory but able to stand pivot transfer, I often will use 30 sec chair rise test and document if they were able to perform with or without UE. I also use SPPB quite often as it’s quick to perform and covers both 5xsts, gait velocity and static balance components (feet together, semitandem and tandem).

Elderly Mobility Scale:
Used for: Function, DC disposition
Test overview: pt is assessed in the following conditions:

  • Bed mobility, standing, walking, functional reach
Scoring: Pt scored per instructions for a maximum of 20 points

Interpretation for function:
<10: dependent in mobility, require help with BADLs
10 – 13: borderline in terms of safe mobility and independence in ADL

14: able to perform mobility alone and safely, independence in BADLs

Interpretation for discharge:

14: home
10-13: home with partial help
6-9: home with high level of help
<6: SNF

Detectable changes for geriatrics:
MDC: 4.3
MCID: 2 points

OTHERS: Gait Speed, Activity Measure for Post Acute Care (AMPAC), Functional in Sitting Test.
